For the benefit of others who may encounter this problem.
One of our users reported that the display of his toolbars in Annotation were all messed up. See the attached JPG image.
He submitted it to CoCreate support and they helped track down the problem. This was caused by a custom toolbar icon (bitmap file) that he had created with a size of 78 x 76 pixels. When he resized it to the standard 24 x 24 pixel size the problem went away. The odd thing is that the problem occurred even if the toolbar containing the odd size icon was turned off.
